Img links & Smiley's

Cannot seem to get images show up that are inserted, ie smileys and pics using the "Insert Image" button. Why is this so?

What are you putting in? Is it html or the image location?

Or use the image inert button & paste the img url.

I looked at what you've typed, and apart from putting the word 'then' in you've also got URL tags in there which you don't need. You can remove these by editing your post, highlighting the image location and pressing the unlink button
I'll edit your second example to show it works. It appears you can't nest some tags.

Ta da! That needs you to go into the Advanced editor to do that. I suspect the reason why it happens is you are copying the actual link to the image rather than its location.
e.g.

http://puffingcigars.com/wp-content/gallery/blond-cigar-smoking/cigar-lady.jpg[URL="http://puffingcigars.com/wp-content/gallery/blond-cigar-smoking/cigar-lady.jpg"]
http://puffingcigars.com/wp-content/gallery/blond-cigar-smoking/cigar-lady.jpg[/URL]

See the difference? You used the second but you needed the first. It happens because OSs are so 'advanced' that they now copy things you can't see. If you still have trouble, paste it into Notepad (assuming you use Windows) first, then copy it again FROM Notepad. It will lose its 'linkification' that way. Hope that helps!
