I don't think your first 2 bolivar are right,
Sure it started in 2007 with bolivar gold medals and ScDLH which there was a few different sizes

The first two were included in a special humidor for LCDH that was limited to 200 pieces. They included 50 of each cigar. The 2004 release of the LCDH Bolivar Gold medal was a special release for German LCDH's limited to 2000 boxes.

There is humidor releases every year there not what are classed as LCDH release

The 2004 gold medals were made for wolters and Germany only like you said so not really a lcdh release thought they did get released later to rest of world. In identical packaging

Worth noting also that lcdh release are not only sold through lcdh for example Spain has none so there sold through shops like normal stock

Yup, but they still are LCDH releases in my book. Both were made for LCDH's and sport the special LCDH band. The Gold Medal 2004 was released by 5th Avenue (the German Habanos distributor back then, now they distribute to Austria and Poland as well) to commemorate their 5th anniversary and distributed exclusively in Germany by LCDH's.
